Patient-Centered Medical Home (PCMH)

Starting with practice evaluation gap analysis and developing a customized timeline for completion of required steps to medical home recognition and concluding with submission of their application documentation.
Becoming a Patient-Centered Medical Home
The PCMH model of patient-centered care sets the stage for various other reform initiatives as it aligns with the “Triple Aim” of improving patient care, improving population health, and reducing healthcare costs.
The Patient-Centered Medical Home (PCMH) practice model has been adopted and recognized by practices nationwide and has been shown to strengthen the clinician-patient relationship while improving the quality of care at a practice.
This collaborative care model has been shown to result in improved practice staff and patient satisfaction, system efficiency, and also may reduce overall practice costs. Various payers in New York State offer incentive payments to providers who meet PCMH criteria.
The National Committee for Quality Assurance (NCQA) is a widely accepted accreditation organization that recognizes practices who achieve the appropriate level of transformation. For those applying for NCQA recognition, NYeC guides qualified practices through the application process and, as a NCQA “Partner in Quality,” helps them to obtain a 20% fee discount.
