HEAL 5 Vendor Contract Language and RHIO Model Consent Form Approved

At the January 29, 2009 meeting of NYeC's Policy and Operations Council, the New York State Department of Health announced that it had approved two other work products emerging from the Statewide Collaboration Process. These items are as follows:

  • Vendor Contract Requirements: This document sets forth the terms and conditions that must be included in contracts between RHIOs and CHITAs that have been awarded HEAL 5 grants and certain of their technology vendors to ensure that any software or services provided by those vendors complies with the Statewide Policy Guidance.
  • Model RHIO Consent Form: This is the model form that RHIOs and their participants must use to collect consumers’ consents for providers to access their medical records through the RHIO. Effective June 1, 2009, RHIOs and their participants must starting using this form or a state-approved substitute for new consents. The full set of privacy and security policies can be found here.
