Mission:
To develop, revise and assess materials that aid ambulatory primary care practices and facilitators in electronic health record (EHR) implementation and workflow redesign. The work group’s aims are to assist office practices in achieving and maintaining meaningful use, to help offices acquire capability and practice expertise as NCQA recognized patient-centered medical homes (PCMHs) and to share their best practices and patient-centric approaches when adopting health information technology.
Objectives:
- Define the key areas/ services/workflows a practice must address when redesigning the medical office as part of an EHR integration process
- Develop education modules that will guide providers/ facilitators in redesign of key physician office processes
- Develop strategies to disseminate, train, monitor and measure use of the redesign tools & modules
- Convene work-group participants in the implementation and testing of work group tools and modules
- Ensure the developed EHR implementation tools support the New York State goals of achievement of meaningful use and National Committee for Quality Assurance patient-centered medical home recognition
Prioritized Projects for 2011:
- Define framework and organization of modules that guide workflow redesign and that support EHR adoption, meaningful use, and the PCMH
- Convene sub-groups (as needed) to focus on modules as required
- Develop approaches to disseminate, train, monitor and measure use of the redesign tools
- Identify and outline methods for collection and dissemination of best practice models for implementation
